What you should have at the appointment:

  • Photo ID (for example, your drivers license)
  • An interpreter (If needed)
  • List of healthcare questions
  • List of other health care providers you have visited and why
  • $30 Payment (due upon arrival)

*Please show up on time for your appointment or cancel with at least 24 hours notice.

*We are committed to ensuring everyone receives care, regardless of their ability to pay. If you are unable to pay the $30 visit fee, you must inform us before your appointment so that we can review your eligibility for financial assistance.
