- Adults aged 18 and older
- Must be uninsured or underinsured
- Income less than 200% of the Federal poverty level
- You must provide gross family earned and unearned monthly income and disclose family size (number of dependents only).
- Those seeking to prevent, manage, or reverse chronic health conditions.
- *Patients are seen by appointment only.
Fees

We offer a sliding fee scale, based on income and household size. Services are provided at no cost for individuals and families at or below 200% of the Federal Poverty Level.

Medications: Discounted.

Routine labs: No charge.

Specialty labs: At cost.
Included:
- Visit with a provider
- Routine labs
- Health education & navigation
- As needed: referrals for mammograms, x-rays, or additional services

Tuesdays: 9am - 2pm
