Community Whole Health provides non-emergency primary/preventive medical care for adults by volunteer physicians, nurse practitioners, physician’s assistants, nurses, and medical assistants.
We provide primary medical care using Lifestyle Medicine interventions as a primary modality to treat chronic conditions including, but not limited to, hypertension, type 2 diabetes, and obesity.
Healthcare Network
We have a network of providers and clinics that perform services outside of our facility, including radiology and imaging services, colonoscopies, and mammograms, etc. Eligibility for these services varies, and you must be a patient currently under care with the clinic to be referred to a service within the Healthcare Network. Health Education
