Patient Eligibility Guidelines
- Adult (18 and older)
- Must be uninsured or underinsured
- Income less than 200% of the Federal poverty level
- You must provide gross family earned and unearned monthly income and disclose family size (number of dependents only)
*Patients are seen by appointment only.
Fees
- All medical visits with a provider: $40 per visit
- Missed appointment fee = $15
- Labs = Discounted
- Medications = FREE
With these fees, our patients receive:
- Visit with a provider.
- Free medications
- Free health education & navigation
- As needed: free mammograms + colonoscopies/endoscopies + referrals for additional services
*Currently our clinic only offers telehealth visits.

- Tuesdays: 9am - 3pm
