Patient Eligibility Guidelines
- Adults aged 18 and older
- Must be uninsured or underinsured
- Income less than 200% of the federal poverty level.
- If you are not working: You must provide proof of unemployment. You can obtain a letter from the Georgia Department of Labor (GDOL)
- Those seeking to prevent, manage, or reverse chronic health conditions.
*Patients are seen by appointment only.
Fees
- All medical visits: $30
- Missed appointment fee: $15
- Medications: Discounted
Included:
- Visit with a provider.
- Routine labs
- Health education & navigation
- As needed: free or discounted mammograms + colonoscopies/endoscopies + referrals for additional services
Clinic Schedule
- Tuesdays: 8am - 3pm
(closed on holidays)
