Community Whole Health provides non-emergency primary/preventive medical care for adults by volunteer physicians, nurse practitioners, physician’s assistants, nurses, and medical assistants.
We provide primary/preventive medical care using Lifestyle Medicine interventions as a primary modality to treat chronic conditions including, but not limited to, hypertension, type 2 diabetes, and obesity.
Healthcare Network
We have a network of providers and clinics that perform services outside of our facility, including radiology and imaging services, colonoscopy, minor and major surgery, mammograms, etc. Eligibility for these services varies, and you must be a patient currently under care with the clinic to be referred to a service within the Healthcare Network. Health Education
